Introduction
A firefighter who is a resident of Alabama is authorized to obtain a “Firefighter” license plate for their personal vehicle. Under Section 32-6-272, Code of Alabama 1975, requires the Alabama Forestry Commission and the Firefighters’ Personnel Standards and Education Commission to prepare lists of persons eligible for distinctive motor vehicle license plates and to authorize the widow or widower of any firefighter who dies in the line of duty to receive a free license plate. Official Alabama Department of Revenue guidelines can be found here.

Fire departments may print these forms on their letterhead if desired. No changes in wording should be made to the form as this is the official form recognized by county license officials for certifying eligibility for the purchase of firefighter tags.
-
It is required that the chief sign the form in blue ink. County tag officials need to ensure that they are receiving the original of the certification form.
-
Volunteer departments: Rosters must be submitted to the Alabama Forestry Commission for tag list eligibility.
